The stage begins on the title screen. Hitting one of the letters causes a different stage hazard to occur. Eventually, the stage transitions into a different section. The Art section features different platforms being painted, stamped, and erased. Different obstacles may also be painted (similar to Pictochat). The Undodog also makes an appearance. The Music section features platforms scrolling to the left, and they may occasionally stop or move back right. Several icons appear and can be interacted with. For example, a Fire Flower will release several fireballs that can damage fighters. The final section, Gnat Attack, features players hopping on different platforms held up by flies. The platforms may move and leave the screen. Other flies will appear, with some carrying items and others attacking the fighters. The fly swatter will also occasionally appear, damaging any fighters it hits and swatting down flies (which will cause them to drop any platforms they may be holding).
I don't think this will happen (at least in the Wii U version) because the Mario series already has a somewhat similar stage (Mushroom Kingdom U). It would still be cool to see a Mario Paint stage, though.
