The obvious flaw in my theory of it being a power-up of some kind is that there are fire versions of some of the doctor characters, and power-ups don't usually stack that way. Perhaps there is a "doctor/fire flower" that grants both fire and doctor abilities.

Sorry, I traveled back in time to the year 1980 and moved a chair in the Nintendo headquarters, and now the Mr. Video Boards seem to be called the Super Mario Boards. At least I was able to prevent Nintendo from being bought by Sony in 2000. That timeline was awful. -
