Sorry for ignoring the request without an answer, but the staff is not approving this name change due to recent controversial events surrounding it. The avatar and signature are fine though. Once again, sorry for the delayed response.

No, it is only your forum name. For example, my current forum name is Deinosuchus, but my username on the wiki is A51 Trooper. If you want to change name on the wiki, do so here.
 
Only if you change your username, but if you change your display name (the name above your profile) you don't have to.
